NEW PATIENT PORTAL: FOLLOWMYHEALTH
EFFECTIVE 11/26/2025: Patients will no longer be able to access their records or interact with us from Patient Fusion. They will need to use their FollowMyHealth account instead. Patients who are currently using or were previously invited to Patient Fusion will automatically receive an invitation to FollowMyHealth via email. Once you accept the invitation and set up your FollowMyHealth account, you will gain access to your clinical data, past and upcoming appointments, and securely message providers. Patients will not be able to message us until they’ve accepted their FollowMyHealth invite and connected their account to our practice.
